This is one of my favorite lessons I did during my 4th grade reading practicum. I printed out mini biographies on various famous baseball players, since my practicum students loved sports so much. My students were able to read all about whichever person they chose, and then filled out a 

"Fakebook" profile about them. I thought it was a fun approach to history which often times can seem monotonous.

Math:

Here are some math worksheets I have created for my 1st grade students. The math worksheets that come with the program we use contain a lot of reading. By making my own, I can accomodate and differentiate to each of my students' needs, and have them focused on the math skill rather than focused on trying to read the directions. The butterfly sheet allows for students to draw in dots if they haven't mastered addition without that manipulative. I also like the sheet in the middle because for lower students I can simply add in the dots in the empty square and they can practice writing the addition problems. The sheet on the far right allows students to get creative with coming up with different parts of 10 and use crayons to illustrate 2 different colored counters.
